There is no purchase necessary to enter Anne Arundel Dermatology’s Meridiem DermSpa, Alchemy Aesthetic Institute, and Dr. McDaniel Laser & Cosmetic Center’s CoolSculpting Model Search. By submitting an entry to this contest, the participant acknowledges and agrees to all of these Terms & Conditions.
*ELIGIBILITY
• Must be 18 years or older to enter
•An in-office assessment must be completed no later than December 30, 2021 to be eligible to win.
• Winner must have treatment(s) by January 31, 2022.
• Winner agrees to document their experience with photos, videos, interviews and posts on various social media platforms.
• Winner will be required to sign Photo/Video Media Release Form. (ie. before & after photos, footage during treatment, testimonial videos, etc.)
• Winner will be required to sign Treatment Consent.
• No cash alternative will be offered. FREE treatments are not transferable.
• If selected winner does not respond to either phone or email contacts made by Meridem DermSpa, Alchemy Aesthetic Institute or Dr. McDaniel Laser & Cosmetic Center to inform him/her of being selected as the winner within 7 days, then another winner shall be chosen, with no further entitlement to the initial winner.
